J-1 Waiver CONRAD State 30 Program

Alabama - As of November 8, 2011 28 slots are available; On October 1st, Alabama began accepting  primary care  applications. Sub-specialty applications will be accepted starting January 1st 2012. Deadline is until all 30 slots are filled.

Alaska - As of November 8, 2011, All 30 Slots are available. Alaska accepts 10 specialty physician applications. No deadline.

Arizona - As of November 8, 2011, Arizona has 30 slots left. Note: Primary Care and Psychiatrists must fill 22 of the 30 slots. Applications are accepted till November 30, 2011 If slots remain available, if slots are still available the cycle is re-opened to more applications. There may not be any specialty slots after the second cycle of applications is finalized.

Arkansas -As of November 8, 2011, 29 slots are available. Applications are accepted on a first come, first serve basis until all 30 slots are filled. Arkansas accepts primary care and specialists applications.

California - As of January 5, 2012, California has 10 slots available. CA accepts applications only for primary care physicians until June 30th. If the slots are not filled by the deadline, specialist physician applications will then be considered. California usually fills all 30 slots with primary care physicians before June 30th.

Colorado - As of November 8, 2011, 30 Slots are available, 9 of which can be FLEX. Colorado has 10 specialty spots available per year. This program accepts applications in a first come, first served until all slots are filled.

Connecticut - As of November 16, 2011, Connecticut has 26 slots available. Connecticut accepts specialty physicians and there is no priority for primary care physicians. This program is first come, first served. They will continue to accept applications until all 30 slots are filled.

Delaware - As of November 8 2011, Delaware has 30 available slots. Applications are accepted until June 30th, 2012. Specialist physician applications will be considered if the facility can document the need for the specialty.

District of Columbia - As of November 8, 2011, 30 slots are available. 10 slots are exclusively for primary care physicians and 20 slots are used for both primary care and select specialty physicians. Deadline is until all applications slots are filled.

Florida - CLOSED- As of November 7, 2011 Florida is no longer accepting applications for the 2011-2012 fiscal year.

Georgia - As of November 8 2011, Georgia has 29 slots available. GA accepts 10 specialty slots each fiscal year with a filing deadline of April 1st. Any other slots remaining unfilled after that day will be available to either primary care or specialist physicians on a first come first serve basis. Applications are accepted until all 30 slots are filled or the start of a new fiscal year.

Hawaii - As of November 15, 2011, Hawaii has 30 slots available. Applications are accepted until all slots have been filled.

Idaho - As of November 8, 2011, Idaho has 30 slots available. Applications are considered from primary care physicians and psychiatrists only. Applications accepted on a first come, first serve basis until all slots are filled.

Illinois -As of November 15, 2011, Illinois has filled all its slots.  Illinois accepts specialty applications; the rules reserve 12 slots however this is dependent on the applications received. Applications will be accepted again from January 1-30 2012.

Indiana - As of November 8, 2011 Indiana has all 30 slots open. Specialists and primary care applications are accepted. Deadline is until all 30 slots are filled or August 1st.

Iowa - As of January 12, 2012, Iowa has 18 slots open. Iowa accepts applications until all slots are filled and accepts 4 specialty physician applications. Deadline is until all slots are filled or Sept 30th.

Kansas - As of November 9, 2011, all 30 Slots are still available. Primary care physicians are prioritized but specialists are considered. No deadline. If an application is received and cannot be processed by the end of the current cycle, it will be processed for the following cycle.

Kentucky – CLOSED- Kentucky is no longer accepting applications for the 2011-2012 fiscal year.  Kentucky’s program accepts applications for each year from Oct 1st to Oct 31st. There is not a limit on specialty physician applications as they are reviewed according to the state’s need.

Louisiana - As of November 15, 2011, LA has 28 slots open. Each year applications for sub-specialists have 6 slots reserved from Oct 1st to March 31st. Applications are the accepted on a first come, first serve basis starting April 1st. However, specialists are required to complete the Dire Need Criteria form with their packets which is then evaluated on an individual basis.

Maine - As of November 15, 2011, Maine has 30 slots available. Unlimited specialty physician applications are accepted, and the deadline to file is until all slots have been filed.

Maryland – CLOSED – As of November 8, 2011, Maryland has filled all slots.

Massachusetts - As of November 8, 2011 all 30 slots are open, Specialists may apply but primary care physicians are prioritized. Specialty applications will be accepted at the beginning of January 2012. Deadline is end of January, 2012 and all applications will be reviewed then. Applications are accepted till all 30 slots are filled.

Michigan - CLOSED - Michigan is no longer accepting applications for the 2011-2012 fiscal year. Michigan does not have a limit on specialty physician applications, but primary care applications receive priority.

Minnesota - As of January 12, 2012, slots are open. Minnesota is accepting applications until all 30 slots are filled or until the deadline of Sept 30, 2011. Specialty applications are accepted and there is no limit, however, the state reserves the right to change this depending on primary care demand.

Mississippi - As of November 8, 2011, 30 Slots are still available. Deadline is until all 30 slots are filled.

Missouri - As of November 9, 2011 Missouri has received 14 applications. Missouri accepts specialty physician applications. Applications are accepted till all 30 slots are filled.

Montana - As of January 12, 2012, There are 30 slots still available.  Deadline is until all slots are filled.

Nebraska - As of November 8, 2011, 28 slots are still available. All slots have never been filled, on average; about 10 slots are filled each year. Deadline is until all slots are filled.

Nevada - As of November 8, 2011 30 Slots are still available. Specialist applications are reviewed on a case by case basis, no defined limit.

New Hampshire - As of November 9, 2011 NH Has 28 slots available. 10 out of the 30 yearly spots can be used as flex spots.

New Jersey - As of November 16, 2011, 20 slots are available. New Jersey accepts applications until all 30 slots are filled or the deadline of September 30 of each year. Slots roll over into the next cycle. Specialty physician applications are accepted for State Designated Trauma Centers and Children Hospitals.

New Mexico - As of February 3, 2012 our office has 7 J-1 Waiver Applications. Please remember that our office accepts applications continuously until we have filled all 30 Waivers. Also, please be aware that the New Mexico J-1 Waiver Program is a first come first serve program.  The New Mexico Department of Health does not reserve J-1 Waiver slots. Applications received after we have filled all 30 slots will be returned.

New York - As of November 8, 2011 there are 30 slots left. New York’s deadline to submit applications is December 1, 2011 and decisions will be made in March 2012. Generally, more specialists as primary care providers are selected.

North Carolina - As of November 10, 2011, 22 slots are available, North Carolina reserves 10 spots for specialty physicians.

North Dakota - As of November 15, 2011, there are 20  slots available  . Applications are accepted year round. There are 10 slots are always filled for physicians that work outside of designated shortage areas. Usually these are specialty physicians although primary care physicians have been placed in these areas.

Ohio - As of November 8, 2011 Ohio has all 30 slots open for applications for the program year 2011-2012. Applications for primary care physicians located in health professional shortage areas will be accepted Oct 1st to June 30th. Applications for primary care physicians not located in HPSAs and all non-primary care physician applications will be accepted Jan 1st through April 30th. Emergent community need applications will be accepted July 1st through Aug 31st. Ohio accepts and does not have a limit on sub-specialist applications.

Oklahoma - As of November 14, 2011, all 30 slots are available. Primary Care is given priority but specialty applications are considered at a case by case basis and 10 applications are accepted for specialty physicians. Deadline to file is until all slots are filled or Sept 30 of every year.

Oregon - As of January 12, 2012, Oregon has 29 slots available. Specialty applications are accepted with 6 slots reserved for Specialty physicians, however that number may change based on the need of the state for the fiscal year. Deadline is until all 30 slots are filled.

Pennsylvania - As of November 8, 2011, PA has 30 slots available. PA accepts applications throughout FFY until all slots are filled. PA accepts Specialty physicians when need is document in an undeserved area.

Rhode Island - As of November 14, 2011, RI has all 30 slots available. Specialty applications are accepted, however the emphasis is placed on Primary care physicians who speak spanish. Applications are accepted till all slots are filled.  

South Carolina - As of November 9, 2011, South Carolina has 27 slots available. Unlimited specialty physician applications are accepted. South Carolina accepts applications until all 30 slots are filled.

South Dakota - As of November 8, 2011, 27 slots are available. South Dakota accepts applications throughout the year and no limits are placed on specialists applications.

Tennessee - As of December 12, 2011, Tennessee has 30 slots available. They have 9 slots available for specialists. Applications are accepted until all 30 slots are filled . 9 specialty applications will be available until March 31st; afterwards all will be accepted as long as the slots are available.

Texas - As of September 19, 2011 TX has filled all slots for the 2011-2012 fiscal year. Applications for the upcoming cycle 2012-2013 will be accepted beginning September 5, 2012. Texas accepts applications until all spots are filled and on a first come, first serve basis.

Utah - As of November 8, 2011, 30 Slots are available. Up to 10 slots are accepted for sub-specialists. Applications are accepted on a first come, first served basis, until all slots are filled.

Vermont – As of November 16, 2011, 25 Slots are available. Specialty physicians are accepted, and there are 4 “Flex Spots.”

Virginia - As of November 17, 2011, Virginia has 29 slots available. Applications are accepted year-round on a rolling basis. Applications processed after September 30, 2011 will be considered for the October 1, 2011 – September 30, 2012 period. Primary care physician and specialty physician applications are accepted but preference is given to primary care.

Washington - As of November 10, 2011, Washington State has 29 slots available. At least 20 are available to primary care physicians and 10 are available each year for specialists. Applications accepted till all slots are filled.

West Virginia - As of November 15, 2011 West Virginia has one slot open. Applications are accepted all year round. Speciality applications are accepted.

Wisconsin - As of November 8, 2011, Wisconsin has 27 slots open. Applications are accepted on a first come, first serve basis until all 30 slots are filled and there is no cap on the number of specialists they accept.

Wyoming - As of January 12, 2012, Wyoming has many slots open. Wyoming accepts 10 specialty physician applications. Applications are accepted until all 30 slots are filled.
